Springfield's Complete Online Path to an IDPH Medical Cannabis Registry Card

Illinois's Medical Cannabis Patient Program accepts physician certifications completed via telehealth. Springfield residents — from the Capitol District and Downtown to Iles Park, Westgate, South Crossing, and every Springfield neighborhood — can complete their entire physician evaluation by secure video call. Your IDPH-certified physician evaluates your condition against the IDPH qualifying conditions list and, if appropriate, submits your written certification through the IDPH provider portal during or after the visit.

Two separate fees — two separate recipients. The physician consultation fee is paid at booking through this platform. The IDPH state government application fee ($50 / $100 / $125 for 1 / 2 / 3 years) is paid separately and directly to IDPH when you complete your registry application at ilogin.illinois.gov. The IDPH fee is non-refundable once submitted.

Print it or save it to your phone and present it with a valid state-issued ID at any licensed Illinois dispensary statewide.
